AI NAVI™ Health Wallet and AI Advocate | RK360®

For senior living: Continuing Care Retirement (CCRC), Home Health, Hospice, Skilled Nursing Facility

Coordinate care for every resident, across every provider, plan, and family caregiver

Your organization gives each resident or patient an AI NAVI™ Wallet, with access for a family caregiver and, if you choose, a care coordinator you appoint. At entry, the Wallet gathers records scattered across past providers, plans, and pharmacies into one place.

As a resident moves from independent living to assisted living, skilled nursing, home health, or hospice, the same Wallet goes along. It proves Medicaid, Medicare, or commercial coverage, and it keeps every clinician and family member working from one current record.

The Senior Living AI NAVI Wallet: one Wallet per resident, distributed by the organization The organization distributes an AI NAVI Wallet to each resident or patient. A Resident Wallet sits at the center with family caregiver access and care coordinator access, inside an organization-distributed, resident-authorized cross-network agentic interoperability layer. Data sources (provider EHRs, plans and pharmacy, devices and documents, voice or photo) flow in; care actions (care plans, coverage proof by QR, shared tasks, alerts for review) flow out. Four steps: 1 Distribute, 2 Gather records, 3 Coordinate the care team, 4 Keep coverage and access. A bottom strip reads: access to care for Medicaid members; proof of Medicaid, Medicare, or commercial coverage; coordinated care for multiple chronic conditions. Why care coordination matters

Uncoordinated care harms seniors, families, and budgets

Most older adults live with several chronic conditions and see many providers who do not share one record. Each handoff is a chance for a missed medicine, a repeated test, a lost result, or a bill no one expected.

79%
of adults 65 and older live with two or more chronic conditions.
1 in 3
adults over 70 leave the hospital with a new major disability.
1 in 5
discharged patients is harmed within weeks. Two in three harms involve medicines.
12 to 34%
of the time, the hospital summary reaches the next doctor before the follow-up visit.
$4,542
more per patient each year when care is fragmented, plus more avoidable admissions.
2 in 3
U.S. bankruptcies involve medical problems: about 530,000 families a year.

What uncoordinated care costs your organization

1 in 4
Medicare patients sent to a skilled nursing facility is back in the hospital within 30 days.
$4.3B
a year is what those returns cost Medicare. Medicare now ties skilled nursing payments to readmission rates.
8%
of market share was lost by one-star nursing homes once star ratings went public. Five-star homes gained 6%.

Early adopter care-coordination pilot

Pilot one Wallet for every resident, configured for your community

A CCRC, home health agency, hospice, or skilled nursing facility can start a pilot with its own residents, care teams, and payer mix. AI NAVI™ treats your intake steps, roles, and forms as settings, not code.

  • Your residents' records, coverage, and care-team roles are set up in the first two months.
  • Day one runs on a browser and QR scans at intake and at each move. EHR and pharmacy connections come later, only where they help.
  • You see records gathered at entry, moves completed with a current care plan, family and outside-provider access, coverage verified at the bedside, and staff time saved.

Schedule a Senior Living pilot conversation

See if you qualify for a care-coordination pilot

Early adopter pilot places are limited, and they go first to organizations ready to act. Tell us what you need now for care coordination and later for cross-network interoperability. If you qualify, we schedule a conversation to scope your pilot.

What early adopters lock in

  • Pricing as low as $1 per patient per year, price-locked for future versions, upgrades, and years
  • Intake, transition, and care-team workflows set up through residents' AI NAVI™ Wallets to fit your community
  • A first say in your care-coordination workflow now, and in cross-network interoperability after the pilot

Organizations that wait join after the early adopter cohort, on the terms in place then.

Pilot pricing applies to qualified pilot partners under a signed pilot agreement and a HIPAA business associate agreement. Terms and conditions apply.
